Registration Process
• Step 1: Register your business either in Office of the Company Registrar as a company or Gharelu Udhyog. Gharelu Udyog or company registration will write a letter to Jana swasthya. You need to take that letter to Jana Swasthya Karyalaya and register the letter.
• Step 2: Go to Janaswasthya, submit all documents and ask them to make a letter to Gharelu or Office of the Company Registrar. Janaswasthya again will give the letter which you need to take back to Gharelu or company registration office.
• Step 3: Go to Gharelu, produce all documents along with the letter made by Janaswathya. According to the investment you have to pay the business establishment charge. Company or Gharelu Udyog registration office will give you a certificate and give you 3 letters. One to Rajaswo, one to Janaswasthya and last one to yourself.
• Step 4: Go to Rajaswo and register your company and also register all the staff in PAN for tax purposes. You will get a certificate for the company and PAN card of all the staffs.
• Step 5: Registered in your ward office (woda karyalaya).
• Step 6: Now go to Janaswasthya Karyalaya and submit all the files. Also list of equipment in clinic and appointment letters of your staffs.
• Step 7: Staffs from Janaswasthya karyalaya will visit your clinic. Once they finish their visit and approve your clinic, pay money the registration charge in Janaswasthya karyalaya.
Required Documents
• Application, Certificates of all staffs and citizenship along with the CVs. And also photocopy of citizenship of all the staffs, house owner, agreement paper with the payment voucher of house tax, electricity bill.
• Photocopy documents of doctors, one CMA or HA, one Staff Nurse, one Lab Technician (Diploma), one pharmacist (Diploma). Make appointment letter to all those staffs.
